Permanent Supportive Housing Program
The Permanent Supportive Housing provides housing to chronically homeless individuals, along with supportive services. To qualify, individuals must be chronically homeless, meaning they must have been homeless for a year or more - or have had 4 or more episodes of homelessness in the past 3 years. In addition, they must have a diagnosable disability (mental illness, addiction, or physical health). While in the Permanent Supportive Housing program, clients receive intensive case management, work on goals and utilize supportive services in order to maintain their housing.
